Spine Physician Institute is referred complex back pain and neck pain patients from across the DFW area. If you’ve been told you need spine surgery, or a spinal fusion, it can be in your best interest to get a second opinion from a regional spine center of excellence. If you bring existing diagnostic studies, including your digital copy of X-rays and MRI.
Use the form to contact the spine center about your spine problem. If you would like to explore setting up an appointment, list the preferred day of the week.
For an initial appointment please arrive 15 minutes before your scheduled appointment. This will allow us sufficient time to complete all of the necessary paperwork. To cancel an appointment, please let us know as soon as possible, and at least 24 hours in advance so we can give that time to another patient who is in pain.
New patients are requested to bring:
- All pertinent medical records, x-rays, MRI and referral paperwork.
- A completed copy of the new patient information sheets mailed to all new patients.

Spine Physicians Institute treats back pain, neck pain, herniated discs, stenosis and other spine problems. Patients come to the spine center from across the Dallas-Fort Worth metroplex. Dr. Sethuraman is one of few Mayo Clinic fellowship-trained spine surgeons in the North Texas area. A fellowship is the highest level of medical education in the U.S.
